Diagnosis of post-surgical recurrence in patients with malignant insulinoma

Buffet A , Vezzosi D , Maiza J-C , Grunenwald S , Bennet A , Otal Ph , Carrere N , Caron Ph

Context: The post-surgical follow-up of malignant insulinomas is often marked by recurrence after initial remission. However, the criteria for the diagnosis of recurrence of these malignant endocrine tumors are not well defined in the literature, unlike diagnostic of criteria for hyperinsulinemic hypoglycaemia (Cryer 2009). We retrospectively reviewed the criteria that allowed the diagnosis of recurrence in four patients with malignant insulinoma after initial post-operative r...

Introduction: Insulin-like growth factor-1 (IGF-I), the main growth factor associated with GH secretion, directly opposes endothelial dysfunction and limit early atherosclerosis by favoring nitric oxide production, promoting insulin sensitivity and preventing postprandial dyslipidemia. A low serum IGF-I level has been reported in patients with OSA syndrome and might be one of the mechanisms underlying cardio-metabolic risks in OSA patients. IGF-1 levels were evaluated in a lar...

Prevalence of acromegaly in patients referred for sleep apnea syndrome (SAS): results of ACROSAS Study

Pepin J L , Galerneau L M , Borel A L , Chabre O , Sapene M , Stach B , Girey-Rannaud J , Tamisier R , Caron Ph

Introduction: Acromegaly is a rare disease (estimated prevalence 40–125 cases per million) resulting from GH/IGF1 hypersecretion, mostly by pituitary adenomas. SAS has a prevalence of up to 80% in acromegalic patients. The high frequency of acromegaly-related comorbidities and delayed diagnosis although effective treatments exist, make it necessary a screening of acromegaly in at risk populations.Aim: In a national multicenter study, to determine th...

Outcome of 59 pregnancies in 43 acromegalic women

Caron Ph , Bertherat J , Brac de la Périère A , Brue Th , Cortet-Rudelli C , Schillo F , Chanson Ph

Limited data are available about pregnancy in acromegalic women. Forty-three women, 32±1 years, presented GH hypersecretion (GH=44±7 ng/ml, IGF-1=1009±82 ng/ml) due to micro (n=6) or macroadenoma (n=37). Thirty-six women had transsphenoidal incomplete surgical resection and 9 external radiation of the pituitary adenoma performed 2.8±0.5 and 7.1±3.3 years before pregnancy. Women received dopamine agonists (n=6), somatostatin analogu...
